The Centers for Medicare & Medicaid Services (CMS) is seeking industry input as it develops a Medicaid Enterprise Systems (MES) IT Standards Program intended to improve consistency across state Medicaid technology environments. The effort focuses on reducing fragmentation, improving interoperability, and supporting modular system implementations through common technical standards.
The RFI describes several challenges CMS has identified across the current MES landscape, including isolated systems, inconsistent technical implementations between states, aging infrastructure, and vendor lock-in. According to the document, these issues increase costs, slow modernization efforts, and make it more difficult for states to implement policy changes. CMS states that the standards program is intended to establish common expectations for data exchange, system integration, and technical architecture while supporting a more modular approach to Medicaid systems.
The agency also states that future standards will align with commercial technologies and existing industry frameworks, including HL7 FHIR and NIST security guidance. CMS plans to implement the program in phases, beginning with an assessment of the current environment, followed by standards development, state implementation support, compliance monitoring, and reporting.
Rather than requesting proposals, CMS is seeking practical input from State Medicaid Agencies, MES vendors, systems integrators, Standards Development Organizations, healthcare providers, and other industry stakeholders. The agency is requesting feedback on interoperability, modular architectures, certification, cloud security, data governance, identity and access management, vendor portability, implementation practices, and technical assistance. The RFI also asks industry to identify considerations for the use of Artificial Intelligence and Machine Learning within Medicaid Enterprise Systems and the data standards needed to support those capabilities.
