VHA and OIT/Office of the Chief Technology Officer are partnering together to solve the following problem: How might we alleviate the burden of doctor-facing clinical reminders in the electronic health record with patient-facing digital questionnaires? Currently, clinicians receive clinical reminders in both CPRS and Cerner, and those questionnaires/screeners are largely completed during the medical encounter. VA does not have an enterprise tool, or series of tools, that allows clinical staff to send digital questionnaires or self-screeners to Veterans.

PROBLEM STATEMENT:

The Office of Primary Care is in search of an enterprise digital questionnaire/self-screener (referred to as “digital screener” going forward in this document) tool that allows customizable and standardized health forms, screeners, questionnaires to be sent to the patient and collect responses asynchronously.

DESIRED USER OUTCOMES

As a Veteran, I can complete the digital screener(s) at a time and place that is convenient to me. Once I complete the digital screener, I receive clear and timely guidance about how to access or update my VA benefits.

By completing the digital screener in advance, the time I spend with my provider is more productive and useful to me.

DESIRED BUSINESS OUTCOMES

The responses from these digital screeners will increase the quality of time the patient spends with the provider by reducing time spent on mandatory documentation.

Digital screening allows the clinician to focus on direct patient care, can satisfy clinical reminders/recommendations, generate health factors, and can push data to various medical teams for measured-based care.

Clinical burden will be reduced through decreased duplicative data entry and reduction in notification fatigue.
