Laurel Health Advisors and the Center for Health and Research Transformation (CHRT) will address the social determinants of health for the Berrien County Health Department

Laurel Health Advisors, LLC, a woman-owned small business located in Laurel, MD, and the Center for Health Research and Transformation (CHRT) at the University of Michigan are pleased to announce a recent award from the Berrien County Health Department to develop a strategic plan to address the social determinants of health (SDOH) that impact the health and well-being of Berrien County residents.  In partnership with the Healthy Berrien Consortium, Laurel Health, and CHRT will develop a strategic plan outlining actionable strategies to address SDOH. This will inform the development of a community needs assessment and implementation plan.

We look forward to collaborating with CHRT and the Healthy Berrien Consortium to identify and develop actionable strategies to address factors outside the health care system that adversely impact individual and community health.”

Key focus areas include affordable care, Medicaid policy, health care issues, coronavirus, long-term care, care transitions, mental health, and substance use.
